Overview

CCHN’s Quality Initiatives Division (QID) provides training, technical assistance, and practice facilitation support to Colorado Community Health Center (CHC) staff to further their clinical and quality improvement (QI) work.

CCHN and Colorado CHCs participate in a variety of QI initiatives and partnerships that support CHCs in providing quality affordable health care.

Providing Support to CHCs

Training & Technical Assistance

CCHN’s Quality Initiatives Division provides training and technical assistance (T/TA) for quality improvement, emergency preparedness, Patient Centered Medical Home (PCMH), payment reform, and clinical topics such as chronic disease and social determinants of health. CCHN provides one-on-one training and technical assistance opportunities to CHCs as well as group TA via webinars. All our work is guided by the needs and input of CHC staff.

Practice Facilitation

CCHN’s Quality Initiatives Division provides practice facilitation (or practice coaching) through a variety of projects including those focused on behavioral health integration, oral health integration, cancer screenings, and implementation of team-based care. Practice facilitation involves working with project champions and a designated team at the CHC to develop work plans, monitor improvement, and advance the goals of a particular initiative.

Peer Support

CCHN’s Quality Initiatives Division supports the following peer groups. Peer groups are position-specific networks for CHC staff to convene and discuss best practices, challenges, and innovations. Peer groups are convened both virtually and in-person.

Partnerships

CCHN’s Quality Initiatives Division maintains several long-term partnerships with state-based and national organizations. CCHN staff participate in task forces and committees for topics related to clinical quality.
