RCHN Cervical Cancer Screening Population Health Initiative
Funded by the RCHN Community Health Foundation, CCHN provides technical assistance, coaching, and practice facilitation to CHCs to improve screening rates and preventative services for female patients in Colorado CHCs, targeting specific interventions and process improvement, including team based care delivery, care coordination, and medical home transformation. CCHN provides coaching to a cohort of CHCs to develop a comprehensive women’s health program, focusing on cervical cancer screening rates, abnormal follow-up, substance use screenings, depression screenings, immunization rates, pregnancy intention, and sexual orientation/gender identity screenings.

Komen Colorado Screening and Diagnostic Project
Funded by Komen Colorado, this project helps improve CHC patients’ health by increasing access to breast health services. Participating CHCs are able to cover costs for direct patient screening and diagnostic services for women who are not eligible for public or private insurance programs. A majority of women served have never been screened or have not been screened in the past two years. In addition to covering costs, CHCs provide necessary breast health education, targeting populations who have the lowest overall screening rates.
